MIDI file must be in the same directory where your .bms/.bme exists. *---EXTENSION HEADER #RANK # *---specifies # ( 0 to 3, where 0 is Very Hard, 1 is Hard, 2 is Normal, and 3 is Easy) as the "rank" of the song.
